Hi. I'm in the process of filling out the various reports I've received this week. One is for chest pain and personal pain, etc. One is for work history for last 15 yrs and one is a function report. While filling out the work history form, I found that there was quite a bit of information already filled out on the form. They have my previous jobs already filled out, what my duties were, times, dates, etc. Is this indicative of anything important? Since they took this info from a previous claim, does that mean they're continuing this as a prior claim? The other forms are blank. I've never seen any information already filled out before so just wondering.

Hi Billie:

When you reapply, they do pull information from your previous claim. I have never heard of the form already being partially conpleted. However, I did have something similar when I reported I started work part-time. I received the SSA-821 and it was partially pre-filled.

Just make sure you agree with the pre-filled information. There were errors in some of the information that was pre-filled on the SSA-821 I received.

Thanks, Pati. Hope you don't mind another questions. I'm in the process of filling out these reports:  Function Report, Pain/Fatigue report and Work Record report. How does one know if they're being detailed enough and stressing their disabilities enough? I don't see anywhere here where they're asking about my congestive heart failure which is the main disability. I've typed up a page about it and will include it in the remarks section of the function report but why were there no questions about it?

I just want to make sure I'm detailed enough and include the information they're looking for. Thanks.

Does the administration here on the forum have time to check out the forms before we send them? I know this is above and beyond your duties here at the forum but I'm just curious if there is anyone who does critique such things. I have the forms filled out but I also have additional information typed out in Word format to attach. These are what I'm wondering if they're detailed enough. I so want to be detailed enough this time.

ETA:  Never mind - this is too much to ask of anyone here. It's just so frustrating trying to get these papers filled out correctly and sufficiently. I evidently botched it 3 times before since I was denied each time and I really didn't want that to happen again this time. And they want the papers back so soon - within 7-14 days. I'll keep plugging away. Thanks!

Hi Billie:

I know how frustrating those forms are. In a sense the Function Report is including your congestive heart failure. It is asking how the totality of your impairments limits your ability to function. Questionnaires for pain, fatigue, etc. are in-house forms used by DDS. They are for documenting conditions that may not be supported by medical evidence. Your congestive heart failure can be supported by medical evidence.

It is difficult to really know how detailed to be as I’m sure each Examiner has their own preferences. I used the Remarks section at the end to go into more detail on specific questions, where there really wasn’t enough room below the question itself. I also included a self-statement that compared the “once me” to the “now me”. Many other people have done the same. It sounds like that is what your extra page. Definitely include it. It is hard to say if it will be used by the Examiner, but it can’t hurt.

Well, the forms are in the mail! I don't hold out much help that I did them well enough but we'll see. I figure the first thing they'll ask for is an echo for a new EF and depending on it is, we'll go from there.
