I would like to thank myaccount for providing this letter to the forum.

It is a request to provide verification of state disability or workers comp benefits.  She received this letter 3 days after submitting her online application.

I truly appreciate all of the wonderful information Dee has provided to the forum.  Letters and forms are one of the best way to learn about this process.  It helps to know what to expect and when to expect it.

I am glad to have been able to help! One point of clarification - this letter also requests SSA-827 (the Medical Release form) and I think I would have gotten it in any case. The reason they requested a copy of one of my state benefits checks is that in the Benefit Application there's a section called "Other Benefits" and in that section there's the question "Filed or intend to file for worker's compensation or other public disability benefits?" I answered that one "yes", since I'm receiving California state temporary disability.

Also on the receipt for that form, it says:

"We will need, and can accept, uncertified photocopies of the following:
   ....
* Award letters, pay stups, settlement agreements or other proof of temporary or permanent workers' compensation-type benefits you received."

It's those things that triggered the inclusion of the request for a copy of my disability check stub in that letter.

Hmmmmm. I think I totally spaced on that part of the application and said no! At the time I didn't know CA State Disability was "public assistance." I did tell the intake rep that I was on disability and it runs out in November. And, I included that in my "self report." Hope this doesn't trip me up somehow? I had no intention of with holding the info. Anyway I expect my last check any day now so I can always make a copy of that and send it in to gather dust with the rest of my file.  :-\

My Account, thanks for sharing the letter. I will never see enough documents from the SSA with their disarming style.  Dragon Fly  I agree with you when you say you will send them a copy of the next check with my  addition of not worrying about it. I believe the information will only be of value to social security under one or two conditions which are: 1. Whether you qualify for SSDI. 2. Whether you qualify for SSDI and or SSI when the medical requirements are met.

For those of you that were on California state disability did you need to pay it back with your back pay from SSDI?

California State Disability is not public assistance.  However, it is income that affects SSI dollar for dollar. For SSDI, it is used in the Public Disbability/Worker's Compensation offset computation.  You cannot receive more than 80% of your Average Current Earnings from the combination of Social Security and PDB/WC for the same month.  So it can affect SSDI, but it may not. Depends on dollar amounts.
