I would like to thank "myaccount" for providing this link to the continuing disability review long form.  It is form SSA-454-BK.  I know a lot of you have asked what is on this form and the kind of information they require.  I am thankful that we can now offer this link so that you can review it and know what to expect should you get the "long form".

Dee, great link. You could also interrupt a lot of the information into completing a initial, reconsideration or ALJ hearing. It shows what they are looking for in a Cont. Disability Review so it could/does also give you some thoughts on what they would look for during filing for initial claims etc. Thanks for sharing Dee!!

Thanks for the long form. I did the short form and I almost decided to get clever with the SSA when they started sending forms and requests of information for my wife. It took the SSA close to 20 years to request my wife to fill out any forms (I concluded that this happened because the SSA was looking at my claim). The SSA would always send a form around every 6 or 7 years and then before I could open the letter good another letter would come from SSA saying disregard the letter there will be no review. Its funny to me when they send the Forms out for my wife. On the last request for information for my wife I ran out diagnoses, I did not know how to categorize a person with a knee replacement that fell and shattered the knew replacement that was about 15 months old, which delayed the other knee replacement and with the mental and her seeing a doctor or two as long as I can remember:Bring it on. But I really really believe your form will help members for years to come as to what to expect and it is quite a contribution.

I'm confused about statements 23/24 about mental hospitalization. Does it somehow look better if I had stayed in the hospital? gee! darn independent living!  sigh

Hello everyone!  First I would like to say that this site is awesome!  It has many helpful people on here and everyone seems to have respect for one another..  I am a disabled veteran and just received a SSA-454-BK (07-2010) in the mail.  I started to fill it out and made a few minor mistakes and would like to revise it a bit but cannot seem to find this NEW CDR form online anywhere.  I went to the link provided above and still no luck.  If anyone knows how to find this form or had it available I would deeply appreciate the help.  Please let me know as soon as possible because I only have 10 more days to have it back to the office.  Thank you all for your time and concern on this urgent matter.

And Welcome.

Unfortunately that is one of many forms not available to the general public. You can only get it directly from SSA, and I highly doubt your local office will even have a copy of it. There are some forms (and I think this is one) that are spit out automatically by computer with your name, SSN, etc autofilled, so there isn't even a paper version available.

If you are able to get a clean, blank form from your local office, would you do us a favor and fax it to us. Our fax number is on our Contacts page. It will come to us in pdf format, and I can create a fillable document for future use.

I also made a mistake on the like 6 question updating medical information form, it was a computer scan form with a barcode, I called s.s. To see could my husband go by the S.S. Office to pick up
A new form as I felt I had ruined the form they mailed me.  They said it was contracted out & there was no way for me to obtain a new form.  I called 5 times & basically got 5 versions of the same story.  The last person told me how to correct  the form and said my phone number was there if  someone had a question.  I was very upset and stressed over it but could not do anything about it.  Each person did say that it happens all the time.

Maybe you had a different form, I just know it is part of the disability that makes my thinking not the best.  I hope you are able to get another form.  I had to accept what I had done and just wait for the results.  I do think it is hard for a lot of disabled people to concentrate on printing the letters and numerals in a way they can be read by the computer.  I was working so hard on printing their way, I lost my train of thought.
